Medical Director

Hospice Care Physician

Our dedicated team of passionate professionals provides the highest quality of palliative care to those with a terminal illness, and a specialized doctor to create and oversee the hospice plan of care. Our hospice doctor is committed to alleviating the patient's symptoms and ensuring a comfortable end-of-life experience.

Director of Patient Care Services (DPCS)

The Director of Patient Care Services assumes various responsibilities, going beyond direct leadership to nursing personnel. Our DPCS is tasked with setting goals for the entire team, creating and updating policies, and coordinating staff members, including volunteers, aides, counselors, and social workers, as well as our administrative support team members. These responsibilities are crucial for ensuring efficient operations and providing comprehensive support to our patients and their families.

Case Manager

  • Primary point-of-contact, meeting patients and families regularly to address their needs.

  • Assessing patient history for personalized care plans.

  • Collaborating with healthcare professionals and other provider agencies for treatment coordination.

  • Maintaining patient records and care information.

  • Providing counseling and support during difficult times.

  • Working with insurance coverage.

  • Recommending appropriate treatments and equipment.

Our hospice team of certified home health aides, hospice aides, and homemakers are devoted to providing compassionate care and support to those in our community facing the end of life's journey. They provide personal care services, such as bathing, dressing, and feeding, under the assessment and supervision of our nurses.

Pharmacy / Pharmacist and DME / Medical Supplies

We operate a fully-integrated pharmacy, stocked with a comprehensive range of medications, durable medical equipment (DME), and medical supplies. Our team of pharmacists works closely with our healthcare professionals to manage medication, provide guidance on drug interactions, and ensure the best possible pain and symptom management, as well as facilitating access to necessary DME and medical supplies.

Therapists

The Hospice of Southern California is proud to offer high quality care with a team of experienced and dedicated therapists. Our therapists provide comprehensive rehabilitative and supportive care depending on the individual needs of the patient and can include occupational, physical, and speech therapy services.
